What Is Apical Resection? Apical resection — also known as apicoectomy or root-end surgery — is a minor surgical procedure used to eliminate persistent infection at the very tip of a tooth root. When conventional root canal treatment cannot fully reach or resolve the infection at the apex, this procedure offers a targeted, tooth-preserving alternative to extraction. During the procedure, the infected tissue surrounding the root tip is carefully removed and the root end is sealed to prevent reinfection. The goal is to save the natural tooth and restore long-term oral health without the need for an implant or bridge.
Who May Need Apical Resection? Your dentist may recommend this procedure in the following situations:

The Treatment Process 1. Diagnosis and Planning High-resolution digital imaging is used to map the exact location and extent of the infection and to assess root anatomy. This allows for precise, minimally invasive surgical planning. 2. Local Anaesthesia The procedure is performed under local anaesthesia. Most patients experience no pain during treatment and report only mild post-operative discomfort for a short period afterward. 3. The Surgical Procedure A small incision is made in the gum tissue near the affected tooth to gain access to the root. The infected tissue is removed, approximately 3 mm of the root tip is resected, and the root end is sealed with a biocompatible material. 4. Healing and Follow-Up Sutures are typically removed within a few days. Complete bone healing takes place over several months and is monitored through scheduled follow-up visits.
